When Pain Persists Despite Treatment: What’s Next?

You’ve done the PT. You’ve seen the orthopedist. You’ve rested, iced, stretched, foam rolled, and followed the protocols. And still… nothing’s changed.

Maybe you can’t get through a hike without your knee flaring up. Maybe your low back tightens up every time you try to deadlift again. Maybe your energy crashes so hard after a workout that you wonder if it’s even worth it anymore.


So you start to think, “Well… maybe this is just my new normal.”


I want to stop you right there.


Because I’ve got some news: This isn’t your new normal. And it doesn’t have to be.

One of the most common things I hear from new patients is:“I’ve already tried everything. It didn’t work.”


They come in carrying test results, imaging, doctor notes, and stories of months (sometimes years) of doing “all the right things” with zero improvement—or worse, feeling like they’re getting left behind while everyone else around them gets stronger.

It’s defeating. It’s isolating. And it leads to this quiet resignation: “I guess I just have to live with it.”

One of my patients came in nearly in tears after months of dealing with sciatica. He’d already seen her primary care doc, tried physical therapy, and was told it was all coming from his lower back. He was doing stretches and exercisese, taking meds, and even tried resting from his workouts—but nothing helped.He was still getting that sharp, radiating pain down his leg whenever he sat too long, stood too long, or tried to run. And he was starting to believe, “This must just be how it is now.”But here’s what we found after just one session:His low back wasn’t the real issue.The problem was coming from his hip.Tight, overactive hip rotators and underactive glutes were creating irritation around the sciatic nerve. Pain Doesn’t Always Show Up Where the Problem Is. Pain is tricky. It's your body’s alarm system—but it doesn’t always tell you where the fire is.

  2. Stop settling for generic answers. “Just rest” or “take these anti-inflammatories” is not a long-term solution.

  3. Get a fresh set of eyes. You need someone who sees the full picture—movement, nutrition, stress, lifestyle—and knows how to connect the dots.

  4. Track actual progress. If you’re not seeing a shift within 1-2 weeks of starting a new plan, it’s time to reassess. We don’t guess—we test, measure, and pivot when needed.
